Enrollment Forms

Thank you for your interest in enrollment at Excelsior! Attached are the required documents to complete your little one’s enrollment, the Supply List, and our Uniform Information. These pieces of information can be submitted via email to enrollments@excelsiorphila.com, or dropped off to the school building on week days between 7 AM & 6 PM. School starts for all Excelsior students on Wednesday, September 9, 2026 at 8 AM. Children must arrive to school no later than 8:30 AM. All of the required enrollment documents must be returned before your child may start school.

If you have subsidy (CCIS or ELRC), you must notify CCIS that your child will be enrolled at Excelsior. Our provider number is 3114076055-1. We also accept the Children’s Scholarship Fund of Philadelphia.

For those parents interested in ordering uniforms online, click here to check out our school’s online school store. For those parents who prefer to purchase their own uniforms and have them embroidered, Smile Kiddie Shop (123 W Chelten Ave, Philadelphia, PA 19144) has our school logo on file. Uniform is not mandatory for preschoolers.

Immunization Records or an Immunization Refusal Form
Dental, Vision, and Hearing Screening for enrolling child
An Emergency Contact Form
Birth Certificate for enrolling child
Insurance Card for enrolling child
Parent/Guardian Federal ID or Driver’s License
Income Verification: two months of paystubs, a W-2, a letter from your employer, or a “No-Income Form“
IEP (If Applicable)
